Please arrive 15 minutes early to ensure that you have enough time to complete the required paperwork.
Bring clothes that do not restrict your movement. No dresses or skirts. Changing rooms are available.
Driver’s license, passport, school ID, or any other form of picture identification accepted.
Please bring any previous diagnostic imaging including X-Rays or MRIs, related to your condition.
Payments for deductibles, co-pays, and uninsured services are due at the time services are rendered. We accept cash, checks, and credit cards.
If applicable, proper insurance documentation helps to expedite the registration process.
